Catchment Youth Refuge

The Catchment Youth Refuge provides essential support to young people and their children in order to address the root causes of homelessness.

Who is the program for?

This program is for young people who are homeless and their children. This includes single young people 16-25 years old, including young parents and children.

What are the objectives of the program?

Catchment Youth Refuge provides safe and secure short-term crisis accommodation and support to young people and families who are accommodated here. The refuge has a very homelike environment that is staffed 24/7 and is located in Inner Melbourne. Workers at the refuge support young people to address issues that may have led to their homelessness and prevented them from sustaining longer-term housing.

Youth and Family Program

The Youth and Family Program has been created to offer case management support to young people who are homeless or risk of homelessness, including locating appropriate accommodation options, as well as provide support surrounding family reconciliation and family conflict. 

Who is the program for?

This program is aimed at young people between 16-25 years of age. The Youth and Family Program provides short to medium-term support to young people who are homeless or at risk of becoming homeless. This program offers assistance to young people in developing independent living skills and improving their lives through early intervention and support.

What are the objectives of the program?

The program has two main areas of support—the first being case management to young people who are homeless or risk of homelessness. The second is about family reconciliation. In the first stream, the worker supports young people to get support and to access appropriate accommodation options. In the second stream, an intensive and targeted approach is provided to address family conflict and issues with the young person, which may include their family.

The Youth and Family Team works closely with the Homelessness Service Sector (HSS) to support young people in improving their relationship with their family through team case management, family mediation, and family conferencing. 

 The program goals are to support young people to:

  • Remain or return home if safe and appropriate
  • Remain connected to family in some capacity
  • Reconcile within themselves through referrals for individual therapeutic counselling and/or group and family therapies.

Referrals can come from a variety of places, access points, schools, youth services, self-referrals.

Housing Support for the Aged Program(HSAP)

The Housing Support for the Aged Program (HSAP) provides long-term outreach case management to individuals who are over 50 and live alone in public housing.

HSAP supports clients who have a history of homelessness or are isolated and living with complex, unmet health needs. Clients are assisted in maintaining their housing through advocacy and support to address unmet health needs through referrals to appropriate health services and to access social support and make connections within their community. Brokerage is available to support the goals of this program.

Who is the program for?

This program is for people who

  • Are over 50 years old and living alone in public housing
  • Have a history of homelessness
  • Have complex unmet healthcare needs
  • Are socially isolated
  • Require case management to maintain their housing and health links

What are the objectives of the program?

The overall objective of HSAP is to prevent homelessness and address social isolation and unmet health needs. We do this by engaging early to maintain individuals’ independence and allow them to remain at home, establish links with the local community, increase social connectedness and belonging, and prevent premature and inappropriate admission to long-term residential care.

Where is the program offered?

The HSAP provides services to residents of Darebin, Moreland, Nillumbik and Whittlesea.

How can you access this program?

The HSAP can be accessed via the following referral pathways:

  • Allied health services

Volunteer Program

The Volunteer and Community Development Program at Barkly Street and Holmes Street aim to provide social interaction and support to residents of these public housing estates. Community activities are designed to engage and connect residents within the community.

MOSS encourages community members from all ages, backgrounds and skill levels to volunteer with the HSAP Volunteer Program. There are numerous volunteer roles that support our residents to engage in a range of enjoyable and relevant activities, while also increasing our resident’s connection to the wider local community.

Transitional Outreach Support Team (TOST)

The Transitional Outreach Support Team provides case management support to people (individuals and families) who are homeless or at risk of experiencing homelessness. The main focus of case management is to support people in addressing the support needs that have led to their homelessness or prevented them from sustaining long-term housing.

Who is the program for?

TOST supports individuals and families living in the North East Melbourne Area (NEMA) catchment of Melbourne’s DHS Northern Metropolitan Region. Many of these consumers reside in transitional housing however, the program also supports consumers who have been assessed as having at-risk or vulnerable tenancies. TOST is partially funded as a Cross Target and partially funded as a Families program. Consequently, it works broadly with males, females, families, individuals and youth.

What are the objectives of the program?

The aim of the program is to work collaboratively with clients on case plans that address issues that have led to their current homelessness period. This support may include referral to specialist services and universal services as well as direct work to facilitate consumers’ ability to develop skills to achieve housing sustainability.

TOST supports consumers in identifying their strengths and resources, whilst recognising options that may assist in obtaining affordable, safe, long-term housing best matching their needs. The Team and respective consumers jointly develop a case plan to achieve their goals. Case management support is also provided to children within families.

Where is the program offered?

North East Melbourne Area (NEMA) catchment of Melbourne’s DHS Northern Metropolitan Region.

How can you access this program?

TOST receives its referrals from Have Home Safe, an Access Point located in Preston that services the NEMA catchment. Homeless people will arrive at Haven Home Safe seeking housing and support around their homelessness issues and undergone an initial assessment. On receipt of a referral, TOST workers will attempt to contact the consumer or nominated contact person to make an appointment and begin the delivery of case management support.

Specific Referrals:

  • TOST – All referrals via the Access Point-Haven Home Safe
  • ICMI – Partnership with WT Onus hostel and any Aboriginal services
  • Banyule – 1 position is funded by Banyule council. Any Banyule Council Service can refer
  • DHHS funded position receives referrals through Access Point
  • Referrals are via email to TOST@Merri.org.au

Intensive Case Management

Intensive Case Management response provides medium-term support and brokerage funds for people with high and complex needs who are experiencing homelessness and find it challenging to access generalist and specialist services. This program is for Indigenous Australians – Families, Singles and Couples with complex needs. Intake takes place after a consultative assessment with the clients, Aboriginal hostel staff and an ICM Worker.

MOSS Planned Activity Groups

The MOSS Planned Activity Groups program is flexible, enabling clients of varying interests, diverse backgrounds and abilities to participate in activities, cultural celebrations and outings in a respectful, safe and welcoming environment.

Social Support Groups are designed to encourage and support individuals to remain socially connected to their communities. Our goal is to enable people to remain living independently, stay engaged in meaningful activities to reduce social isolation and improve their general health and well-being through physical and mental stimulation.

Who is the program for?

This program is for:

  • NDIS participants
  • Clients 55 years and over
  • Indigenous Clients 50 years and under

What are the objectives of the program?

Clients are encouraged to identify their individual recreational and social goals and will be supported to achieve their desired outcomes through meaningful and supported planning. We assess each person’s abilities and interests and incorporate them into the modern world. In our rooms, we wish to create a space where each attendee is accepted, respected and made to feel welcome by the group. We want participants to feel supported, but we also like to challenge them to learn new skills.

Social Support Groups provide a friendly and inclusive environment for the aged and individuals with disabilities. We identify individuals’ needs and develop social and recreational goals to support each client’s abilities. Nutritious meals are provided with each session considering client input and dietary requirements. Many of the wonderful activities and cultural celebrations often revolve around food throughout the year.

Where is the program offered?

Social Support Groups are located in the local government areas of Moreland, Darebin and Banyule. Our Planned Activity Groups are located on the Office of Housing Older Persons estates in spaces shared by tenants to varying degrees. As such, these groups provide a drop-in service to persons living on these estates and may not be current clients of the program. Programs are developed to be welcoming and friendly to foster a sense of community connection and address social isolation issues whilst involving participants in the planning of activities.
