The Darebin Assertive Community Outreach (DACO) program provides assertive outreach case management, development of case plans and assistance finding support to address some of the issues that have led to consumer homelessness.
Who is the program for?
DACO is for homeless people in the City of Darebin. This program tries to get people who are sleeping rough off the street and into accommodation.
What are the objectives of the program?
The DACO program is a MOSS program in partnership with Darebin City Council. The program provides proactive outreach case management to assist people who are homeless and sleeping rough in the City of Darebin through:
- Actively seeking out and engaging rough sleepers using assertive outreach
- Providing short to medium-term case management and linkages to other services
- Providing referrals to various housing and accommodation options
- Providing referrals to general and specialist services
