Hume Transitional Support Program

The Hume Transitional Support Program provides medium to long term transitional housing support. The core work of the program is team case management. The program helps people to address their support needs and barriers to maintaining and sustaining housing.

The MOSS-Hume Transitional Support Program helps the consumer develop a case plan that addresses these barriers through effective linkages and support with capacity building.

Who is the program for?

The Hume Transitional Support Program is for people experiencing or are at risk of homelessness. This includes singles, couples and families who have been assessed as being homeless, or at risk of becoming homeless and whose links are within the City of Hume or the City of Moreland.

What are the objectives of the program?

The program’s critical role is in assisting consumers in addressing the barriers to obtaining and, more significantly, maintaining, appropriate accommodation. The program makes referrals to services that are identified as appropriate for the individual.

Where is the program offered?

This program is offered in the Hume and Moreland LGA’s.

How can you access this program?

Referrals for the program are received from the local Access Point – VincentCare Housing in Glenroy.

Support Programs

Transitional Outreach Support Team (TOST)

The Transitional Outreach Support Team provides case management support to people (individuals and families) who are homeless or at risk of experiencing homelessness. The main focus of case management is to support people in addressing the support needs that have led to their homelessness or prevented them from sustaining long-term housing.

Who is the program for?

TOST supports individuals and families living in the North East Melbourne Area (NEMA) catchment of Melbourne’s DHS Northern Metropolitan Region. Many of these consumers reside in transitional housing however, the program also supports consumers who have been assessed as having at-risk or vulnerable tenancies. TOST is partially funded as a Cross Target and partially funded as a Families program. Consequently, it works broadly with males, females, families, individuals and youth.

What are the objectives of the program?

The aim of the program is to work collaboratively with clients on case plans that address issues that have led to their current homelessness period. This support may include referral to specialist services and universal services as well as direct work to facilitate consumers’ ability to develop skills to achieve housing sustainability.

TOST supports consumers in identifying their strengths and resources, whilst recognising options that may assist in obtaining affordable, safe, long-term housing best matching their needs. The Team and respective consumers jointly develop a case plan to achieve their goals. Case management support is also provided to children within families.

Where is the program offered?

North East Melbourne Area (NEMA) catchment of Melbourne’s DHS Northern Metropolitan Region.

How can you access this program?

TOST receives its referrals from Have Home Safe, an Access Point located in Preston that services the NEMA catchment. Homeless people will arrive at Haven Home Safe seeking housing and support around their homelessness issues and undergone an initial assessment. On receipt of a referral, TOST workers will attempt to contact the consumer or nominated contact person to make an appointment and begin the delivery of case management support.

Specific Referrals:

  • TOST – All referrals via the Access Point-Haven Home Safe
  • ICMI – Partnership with WT Onus hostel and any Aboriginal services
  • Banyule – 1 position is funded by Banyule council. Any Banyule Council Service can refer
  • DHHS funded position receives referrals through Access Point
  • Referrals are via email to TOST@Merri.org.au

Intensive Case Management

Intensive Case Management response provides medium-term support and brokerage funds for people with high and complex needs who are experiencing homelessness and find it challenging to access generalist and specialist services. This program is for Indigenous Australians – Families, Singles and Couples with complex needs. Intake takes place after a consultative assessment with the clients, Aboriginal hostel staff and an ICM Worker.

MOSS Planned Activity Groups

The MOSS Planned Activity Groups program is flexible, enabling clients of varying interests, diverse backgrounds and abilities to participate in activities, cultural celebrations and outings in a respectful, safe and welcoming environment.

Social Support Groups are designed to encourage and support individuals to remain socially connected to their communities. Our goal is to enable people to remain living independently, stay engaged in meaningful activities to reduce social isolation and improve their general health and well-being through physical and mental stimulation.

Who is the program for?

This program is for:

  • NDIS participants
  • Clients 55 years and over
  • Indigenous Clients 50 years and under

What are the objectives of the program?

Clients are encouraged to identify their individual recreational and social goals and will be supported to achieve their desired outcomes through meaningful and supported planning. We assess each person’s abilities and interests and incorporate them into the modern world. In our rooms, we wish to create a space where each attendee is accepted, respected and made to feel welcome by the group. We want participants to feel supported, but we also like to challenge them to learn new skills.

Social Support Groups provide a friendly and inclusive environment for the aged and individuals with disabilities. We identify individuals’ needs and develop social and recreational goals to support each client’s abilities. Nutritious meals are provided with each session considering client input and dietary requirements. Many of the wonderful activities and cultural celebrations often revolve around food throughout the year.

Where is the program offered?

Social Support Groups are located in the local government areas of Moreland, Darebin and Banyule. Our Planned Activity Groups are located on the Office of Housing Older Persons estates in spaces shared by tenants to varying degrees. As such, these groups provide a drop-in service to persons living on these estates and may not be current clients of the program. Programs are developed to be welcoming and friendly to foster a sense of community connection and address social isolation issues whilst involving participants in the planning of activities.

Connections Programs

The Connections Program offers short-term support to singles and couples over the age of 26, who live in Melbourne’s northern suburbs, with multiple or complex needs and who are homeless or who live in low-cost accommodation.

Who is the program for?

The Connections Program works with people who are experiencing both

  • Unmet health needs and
  • Homelessness, housing issues, or living in low-cost accommodation (including social or public housing)

The Connections Program also supports people who may:

  • Have few links to other services
  • Need support and to gain skills to live independently
  • Are isolated with little social or family support
  • Have complex support needs
  • Live in unsafe housing
  • Experience hoarding and squalor
  • Have difficulty making or attending appointments
  • Have difficulty finding the right services
  • Need links to health and community supports

What are the objectives of the program?

We connect people to the services and supports they require. Through providing these links we hope to assist our clients with finding the proper support and guidance needed to overcome barriers.

Where is the program offered?

A vital role of the Connections Program is to support people to link to the appropriate services.

Our catchment areas are:

  • Banyule
  • Whittlesea
  • Darebin
  • Nillumbik
  • Brunswick and East Brunswick

We also support people over 55 years of age living in low-cost accommodation in:

  • Hume
  • Moreland

We accept referrals from all services, families, friends and individuals.
