Support Coordination

Merri Outreach Support Service (MOSS) understands the impact of homelessness and housing insecurity on people with disabilities.

We understand that having nowhere to live, staying in rooming houses, couch surfing, and unsuitable private rental can create difficulties in all aspects of your life. People with disabilities face greater challenges when they are impacted by any of these housing circumstances and their health and wellbeing can decline as a result. MOSS specialises in providing support coordination to NDIS participants who are experiencing homelessness, unsuitable housing or housing vulnerability.

We do not have accommodation; however participants may be eligible to access existing MOSS programs to assist them to access long term and secure housing. Our Connections Program team members can assist you to explore your housing options and link you to services that can help with your health and wellbeing whilst our support coordination service focuses on getting you the most from your NDIS package.

You are eligible for support coordination with Merri Outreach Support Service if you:

  • Do not have dependent children
  • Are 25 years of age or older
  • Live in, or are linked to, the Hume, Moreland, Darebin, Nillumbik, Banyule or Whittlesea areas
