Holmes Street Older Person’s High Rise Support Program
The Older Persons High Rise Support Program (OPHRSP) responds to tenants living in older person’s low-cost high-rise accommodation who have unmet support needs. Through a proactive outreach model the program engages with, assesses and links tenants into relevant health and community services in order to benefit their health, social connectedness, and to stabilise their accommodation circumstances.

The Older Person’s High Rise Support Program at Holmes Street aims to: improve tenants’ health and well-being; ensure physical, personal and home care needs are being met; assist tenants with social and community access; improve tenants’ access to services; assist tenants to maintain stable and long term public housing; and increase tenants’ involvement in community activities.

Individual support is available to assist Holmes Street tenants in the areas of their lives where they may require support and can include: assessment of individual needs; referral to specialist services, including CAPS packages; assistance with mental and physical health issues; information, advice and support; advocacy support; social and recreational support. Community activities are offered at Holmes Street with the aim of working together with tenants to create opportunities for social interaction and enhancement of the community environment, primarily through group activities. Currently, tenants are able to participate in: Fruits in the Foyer morning tea (Wednesdays 10.30am); the Community Garden Program; community lunches and barbecues; bequest; the Bike Program; health promotion activities.

Eligibility Criteria
The OPHRSP is available to all tenants who live at the Holmes Street estate.
