Housing Support for the Aged Program (HSAP)

The Housing Support for the Aged Program is a long term case management program which provides low level monitoring, care coordination, practical assistance and social linkages. Significant brokerage is available to support the goals of this program.

The overall objective of the program is to maintain the independence of individuals to assist them to remain living at home and to prevent premature or inappropriate admission to long term residential care.

Eligibility Criteria

  • People who are over 50 years old and living in public housing and;
  • Have a history of homelessness and;
  • Have complex needs such as – physical/intellectual disability, mental/physical illness, Acquired Brain Injury, frailty and /or drug or alcohol dependence and are;
  • Socially isolated and;
  • Require case management service to maintain their housing and health links.

Referral

  • Referrals pathway is through the MOSS Connections Program

Catchment Area

  • HSAP provide services to residents of Moreland, Darebin, Whittlesea and Nillumbik
  • Residents of other Local Government areas will be advised of the contact details for their local Housing Support for the Aged Programs
