Intensive Case Management

Intensive Case Management response provides medium term support and brokerage funds for people with high and complex needs who are experiencing homelessness and find it difficult to access generalist and specialist services.

Eligibility

Indigenous Australian – Families, Singles and Couples with complex needs

Referral

Intake takes place after a consultative assessment with client(s),Aboriginal hostel staff and ICM Worker.
