CRISIS RESPONSE PROGRAM

The Crisis Program provides Outreach Case Management support with people who are homeless or at risk of homelessness in the Northern Suburbs of Melbourne.

The Crisis Program incorporates three streams of crisis support:

Crisis Response Program
The Crisis Response Program is a short-term program that offers team case management to support people experiencing homelessness and/or housing crisis. For those consumers requiring longer-term case management we will seek to refer to the appropriate case management program, often a transitional support program. A comprehensive assessment will support the development of a case plan in partnership with the consumer. The case plan will focus on the consumer’s needs and support issues.

Eligibility:
Singles, couples and families who are homeless or in housing crisis. The service is essentially outreach and the team works with people in a range of accommodation settings.

Must be referred by the Access Point Haven; Home, Safe (formerly North East Housing Service)

Youth and Family Program

The Youth and Family Team is funded as a Family Reconciliation Mediation Program (FRMP), part of a state-wide program which was established in 2005 through the Youth Homelessness Action Plan (YHAP) for young people aged 16 – 25 years experiencing or at risk of homelessness.

The Youth and Family Team works closely with the Homelessness Service Sector (HSS) in supporting young people to improve their relationship with their family through team case management, family mediation, family conferencing, individual counselling, group and family therapies. The program goals are to support young people to:

  • Remain or return home if safe and appropriate
  • Remain connected to family in some capacity
  • Reconcile within themselves through therapeutic interventions

Eligibility:
Young people 16 – 25 years of age who are homeless or at risk of homelessness

Referrals:
Referrals are received from a range of sources including access points (THMs), homelessness support services, self-referrals, schools etc.

Catchment Youth Refuge

Catchment Youth Refuge provides safe and secure short-term crisis accommodation and holistic team case management to homeless single young people, including young parents and (their children) aged 16-25 years old. The refuge has a very homelike environment that is staffed 24 hours and is located in Inner Melbourne. Workers at the refuge support young people to address issues that may have led to their homelessness and prevented them from sustaining longer-term housing.

Eligibility:
Single young people 16-25 years old, including young parents and children.

Referrals:
Must be referred by the Access Points which include Melbourne Youth Support Service (MYSS) and Haven; Home, Safe (formerly North East Housing).
